What You Will Do

As an Associate with Seton Family of Hospitals-TX, you will have the opportunity to investigate, assessand planinterventionto help patients cope with social, emotional, economic and environmental problem.

Responsibilities:

  • Interviewpatient, assesspriorities and documentcase activity.

  • Consultwith team manager, statutory and voluntary agencies and patient's relativeto ensure compliance with patient's treatment plan.

  • Assistclient/familywith obtaining community assistance by referral to proper resource.

  • Developand maintainworking relationshipwith community health, welfare and social agencies and seekcreative means to assist the client with his/her needs.

  • Supportmanagement of concurrent review, discharge planning processes, social services, home care and other post discharge need.

What You Will Need

Licenses/Certifications/Registration:

  • Required Credential(s):

  • Social Workercredentialed from theTexas State Board of Social Worker Examinersobtained prior to hire date or job transfer date.

Education:

  • Diploma

  • High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required

Work Experience:

  • 3 years of experience preferred.
